Meet your Care Coordinator

I’m Ciarra Jackson, owner and Care Coordinator. I help families research resource options, put practical next steps in writing, and organize family responsibilities through phone and email support.

What support looks like

Resource research may include home care agencies, adult day programs, transportation, and meal delivery. Depending on the service you choose, support may include a personalized written resource plan, shared calendar setup, task tracking, or a conversation about caregiver stress and boundaries.

Family Check-In Calls offer social phone conversations and written family summaries. These are social check-in calls only and do not include assessment, monitoring, health questions, or care decisions.

Clear expectations from the start

All services are provided by phone or email only—no in-person visits, home visits, or video calls. The person receiving care must live in Virginia. Family members arranging services may live anywhere.

Your family chooses its providers and makes its own arrangements. Grace-Way does not accept referral fees or guarantee provider acceptance, availability, waitlists, or timelines.
